Earlier versions of sections of this monograph have appeared as journal articles and a book chapter. The monograph revises, expands and integrates these earlier works within a single integrated project and adds entirely new chapters, the latter making up the bulk of the book. Entirely new chapters are 1, 2, 5, and 6.
