This is the final version of the protocol for the Horizon 2020 funded EmERGE study (Work Package 5), coordinated from Brighton and Sussex University Hosptitals Trust from 2015-2019. JW, LA, JB, AL and FG conceptualized and designed the study; VC carried out the literature search; JW, LA, JB, IB, MB, DG, AL, ML, GR, ET, GW, SZ, FG collected and prepared the data; SB, JW, JB, MB, LA and FG verified the underlying data; CJ analyzed the data; CJ prepared the figures and tables; VC, JW, CJ wrote the initial draft of the manuscript.
